BMI and Type 2 Diabetes Risk: The Connection

The dose-response curve

Diabetes risk rises gradually starting around BMI 23, climbs steeply through the overweight range, and is very high above BMI 30. A meta-analysis of over 2 million adults found type 2 diabetes risk roughly doubles for each 5-point BMI increase above 22.

Risk is not destiny. Even at high BMI, sustained 5–10% weight loss can prevent or delay diabetes onset — that finding is one of the most reproducible in clinical research.

Why ethnicity changes the cutoff

South and East Asian populations develop type 2 diabetes at substantially lower BMIs than European populations. The WHO recommends Asian-specific cutoffs of BMI 23 for overweight and 27.5 for obesity for diabetes screening.

What to track alongside BMI

Waist-to-height ratio (waist circumference ÷ height) — should stay below 0.5 — flags abdominal obesity, the type most linked to diabetes risk.

Fasting glucose and HbA1c are the actual diagnostic numbers. A yearly checkup catches prediabetes early when reversal is easiest.

A weight loss target that matters

The Diabetes Prevention Program — the largest US prevention trial — showed that 7% body weight loss combined with 150 minutes of weekly exercise reduced type 2 diabetes incidence by 58%. That is a bigger effect than most diabetes medications.
